NFT mint not working & Transaction failure

I just tried minting an NFT from a new wallet, and it not only failed, but somehow created a fake mint also, or at least that's what it looks like. It happened twice, on two different NFT Factories.

Both times I tried, this message appeared, but I'm not sure why.

The weirdest thing, is that it appeared to have minted something in my wallet, but ultimately didn't. Afterwards, I then tried to mint from one of the NFT factories with my main wallet, and it did work.

So I sent the nft to the new wallet, which shows the same NFT, both #19.

I looked into it, and while I don't think the blurred one was minted, I thought it should at least not appear in my wallet.
